Any guidance on getting a more stable OC?

Hey guys, I have read Dolk's Guide and think that I have a decent grasp on the material and tried to get a stable 4.0GHz OC using the FSB and changing all the settings instead of going with the raised multiplier because I couldn't find success with it before. I don't know what the problem is, so I was wondering if you could spot something I'm missing. The motherboard I'm using is the Asus M4A78T-E and the CPU is the AMD Phenom II x6 1090T. My current settings get me into Windows, but I crash a few minutes into Prime:

 

CPU Multi: 16

FSB: 250MHz

RAM: 1667MHz (Also, board only supports up to 1600MHz, is this still okay? I just won't get any extra performance out of it right?)

Timings: 9-9-9-24

CPU/NB: 2500MHz

HT Link: 2500MHz

CPU Voltage: 1.45v

CPU/NB Voltage: 1.25

RAM Voltage: 1.5v

HT Voltage: 1.26v

 

Thanks guys, I am not just trying to get an overclock out of you guys, I am looking for any places that look shaky to you since I am very new to this.
