OC fails with 3rd monitor plugged in

Any idea why my system would revert to fail safe defaults when I plug a monitor into my 4550? The BIOS resets but it boots fine with nothing plugged in, Windows detects the 4550 and all. A couple months ago I removed the card from my system and at that point it was working fine. Setting my vcore to 4 volts usually solves my problems but it isnt working here.
