It really isn't as bad as it looks....presuming that the guy doesn't get a horrible infection, then all they'd do is clean up the edges and close it over a period of time... after a while, the only thing that'd remain would be a small scar where the horn went in....painful, kind of, but not really to the extent of what you'd think. He'd only really run into problems if the knee joint was compromised.
