Video Card issue or Driver?

So i have been using EVGA GTX 580 for the past 2 years or so. Recently with the latest upate of BF4 or origin update (who knows) I have been getting this error : DirectX function "Get Device Removed Reason" blah blah..

 

Googling around people have suggested to remove driver and go into safe mode and use drive sweeper. So drive sweerper has been replaced display driver uninstaller..

 

I download the latest driver.. Uninstall the old driver, safe mode and use DDU to remove the old driver registries and rebboot.. Install the new one and reboot.. Once it reboots, I see the bios screen, windows logo and after that the screen goes blank an after few mins, the screen goes into sleep mode.

 

This has happened before and it worked fine after 1 day.. But it happened again yesterday and it always happen after I get the directx error message..

 

Any ideas? Thanks as always..
